About

Hoi Yan Jasmine Chan
(Registered Social Worker, OCSWSSW, Canada)

Jasmine is a registered social worker providing counselling and psychotherapy. She is a Clinical Fellow of the American Association of Marriage and Family Therapy (AAMFT) and brings over 25 years of experience in counselling.

She began her career after graduating from the University of Hong Kong in 1999, working as a social worker with families, youth, and children from lower socioeconomic backgrounds through times of transition, struggle, and crisis. Over the years, Jasmine has supported thousands of clients. She spent 18 years at a family service centre in Hong Kong, where she also served as Assistant Director for three years, supervising social workers. She later relocated to Toronto to continue her psychotherapy practice.

Jasmine specializes in supporting clients experiencing depression, anxiety, complex trauma, and relationship challenges, including couple, parent-child, intergenerational, and family dynamics. Her approach is compassionate, mindful, and relational. She works primarily from an Emotionally Focused Therapy (EFT), trauma-informed, and family systems perspective, and offers  Eye Movement Desensitization Reprocessing (EMDR) as a distinct, evidence-based treatment for trauma.

Since 2016, Jasmine has been teaching Mindfulness-Based Cognitive Therapy (MBCT) and is a Stage 5 teacher with Oxford Mindfulness (UK).

In addition to her clinical work, Jasmine provides clinical supervision for psychotherapists and counsellors. As an AAMFT Approved Supervisor Candidate working under the mentorship of an Approved Supervisor, she supports clinicians in their professional development, their pursuit of AAMFT Clinical Fellow designation, and their ongoing clinical growth.

In her personal life, she enjoys family moment with her two teenage children. She finds hiking and cooking to be therapeutic activities that help rejuvenate her inner well-being.

She is open to incorporating clients’ spirituality or faith into therapy at their request. If you are seeking personal growth, insight, reflection, relief from suffering, or a deeper reconnection with your inner self and loved ones, Jasmine is here to support you.

The Counseling Process

You are welcome to inquire about my services through a free 15-minute consultation session before scheduling a counseling session. This consultation can be booked through the booking page on my website. Once you book the session, I will send you an intake and consent form as required by the regulatory college. You may complete and sign the form beforehand, or we can discuss it further during the consultation.

After the consultation, I will proceed with the first counseling session. During this session, I will review your intake and consent forms, focus on understanding your current concerns, and work with you to identify goals.

The therapeutic process begins with the assessment, which is an integral part of therapy itself. Many clients find relief in being listened to in a safe, trustful, and confidential environment. The assessment process typically takes place over the first two sessions. During this time, I will explore your past history and family background, identify your strengths and challenges, and gain a comprehensive understanding of your concerns. From this, I will collaborate with you to develop a personalized plan and direction for our work together. I will also discuss the frequency of counseling sessions with you during this phase. I will also discuss the frequency of counseling sessions with you during this phase.

In subsequent sessions, we will work collaboratively toward achieving the agreed-upon goals. As counseling progresses, I will review and adjust goals and plans as needed to ensure they continue to meet your evolving needs.
